Exam services include but are not limited to extended time, reduced distraction, read or scribe services, large print, and or assistive technology.
Exam or quiz accommodations for exams or quizzes given in the classroom
Instructors will submit their schedule and proctor guidelines to DSS directly, and it is the student’s responsibility to contact DSS to schedule in-class or proctored online exams or quizzes one week prior to the exam/quiz date in class. The Instructor is NOT responsible for scheduling the exam or quiz with DSS. Students who choose to utilize their exam or quiz accommodations will be scheduled to take the exam or quiz in DSS during the regular exam or quiz time. If an adjustment is needed due to the availability of exam rooms or back-to-back classes, DSS will contact the Instructor to discuss options. If a student misses a scheduled exam or quiz, it is the student’s responsibility to request permission from the Instructor for an extension. DSS will not reschedule unless given authorization from the Instructor.
STUDENT RESPONSIBILITY
The student is responsible for contacting DSS to schedule their exams or quizzes one week before the class exam or quiz date. If a student misses a scheduled exam or quiz, it is the student’s responsibility to request permission from the Instructor for an extension. If a DSS student does not wish to utilize their accommodations or fails to schedule with DSS, the student may be in class for the exams or quizzes.
INSTRUCTOR RESPONSIBILITY
The Instructor is responsible for submitting the Schedule & Proctor Form to DSS for each exam or quiz one week before the exam/quiz. The Instructor is also responsible for raising the exam or quiz to DSS located in the Central Academic Building, Suite 210, or to dss@tamusa.edu 24 hours prior to the scheduled exam or quiz. When the student contacts DSS to schedule, the appointment will remain pending until the Instructor submits the Schedule & Proctor Form confirming the date and time and proctor guidelines for the exam or quiz. DSS will not proctor an exam or quiz if we have not received the Schedule & Proctor Form. Suppose a student is unable to take a scheduled, accommodated exam or quiz in DSS because the Instructor has not submitted the Schedule & Proctor Form. In that case, the Instructor is responsible for providing an extension to the student for a rescheduled appointment.
DSS RESPONSIBILITY
The Instructor and student will receive an email appointment when a student schedules an exam or quiz. If the Instructor has not yet submitted the Schedule & Proctor Form, the appointment will be titled “Pending.” DSS will send email reminders to the Instructor requesting the Schedule & Proctor Form and exam or quiz. If an adjustment is needed due to the availability of exam rooms or back-to-back classes, DSS will contact the Instructor to discuss options. DSS will administer and proctor the exam or quiz utilizing the Schedule & Proctor Form provided by the Instructor. All completed exams or quizzes will be returned to the department office within one business day.
Exam accommodations in an online format
Once an instructor receives a Letter of Accommodation for a DSS registered student, it is the INSTRUCTOR’S RESPONSIBILITY to adjust the length of the exam or quiz on Blackboard for all online exams or quizzes during the semester. The amount of time the student has been approved is located on the Letter of Accommodation under the exam accommodation section. Below is a list of Instructional Technologists who may assist you with this process. Suppose the online exam or quiz requires proctoring and will be scheduled in the DSS exam center. In that case, Instructors must submit a Schedule & Proctor Form for each exam or quiz along with the necessary access codes to the online accommodated exam or quiz.
