- Download the Outside Employment disclosure form here
- Submit a completed form annually by September 1, 2022 even if you have no outside employment to disclose.
- You must also resubmit the outside employment form within 30 days of acquiring an SFI.

When is the Annual Disclosure due?
Annual disclosures must be completed:
- Within 30 days of the investigator’s initial employment date; or
- Annually, no later than August 31st.
Will COI review delay your research process?
It is important to submit your disclosure and any updates in a timely manner, as the Office of Grants and Contracts (OGC) will not release funds for a project until all compliance reviews have been completed.
If required, COI Committee disclosure reviews can take an extended period of time due to the committee’s docket and the complexity of submitted information, as well as any follow-up meetings or information collection required.
In addition, if you will be using human participants in your research, please note that the Institutional Review Board (IRB) will not complete its review of your application until the FCOI has been submitted.
